By adopting recovery model with medical-social collaboration, we provide multi-disciplinary and transitional support to facilitate service users to bridge community support service and reintegrate into the community.
Lighthouse Project - Pilot Project on Transitional Support Service for Persons in Mental Recovery
Objectives
- To provide support to potential service users of HWH, to facilitate their adjustment in the community, to enhance their understanding in HWH living and prepare their admission, or sustain their community living.
- To provide support to HWH users who left HWH pre-maturely on their integration in community living and to maintain their motivation for recovery.
- To assist HWH service users who have been residing for 36 months or above to be smoothly discharged from HWH and reintegrate into the community.
Service Districts
Wong Tai Sin, Kowloon City, Yau Tsim Mong, Shum Shui Po, Tsuen Wan, Kwai Chung (selective parts) and North Lantau Island
Target Service Users
Persons in mental recovery who are living in the serving districts, including:
- Potential service users of HWH;
- HWH services users left HWH service pre-maturely; and
- HWH service users who have been residing for 36 months or above and have difficulties in discharge.
Scope of Services
Feature of the Project
Multi-disciplinary team, including social worker, occupational therapist, psychiatric nurse, peer supporter and home care worker, provide individual, group and outreaching intervention.
Fees and Charges
Services in the scope of the Project under the Funding and Service Agreements are free of charge.

Service Entry
Target service users and/or their family members/ carers can apply service by self-approach (with prior telephone appointment)
Exit of Service
Participation in service is voluntary. Service users may exit from the service under the following conditions:
- Completion of agreed service plan by service users;
- Service users can apply for termination of service with their caseworkers. Referrals to other services can be provided where necessary; or
- Service users are untraceable or persistently unable to receive the service for more than 8 weeks.
